If you currently do not have a regular family health care provider, we will make every effort to match you with a delivery provider that will also be able to accept you into their Family Medicine practice post-delivery.
Health Care Connect is a program of the Ministry of Health and Long-Term Care to help Ontarians without a family health care provider find one. The program refers people without a regular family health care provider to family physicians and nurse practitioners who are accepting new patients in their communities.
