The Discharge Planning function is carried out by the Patient Services Coordinator. The Patient Services Coordinator is responsible for implementing timely discharge plans that enhance continuity of care and efficient use of hospital and community resources. She also recommends options for continuing care and refers the patient to accommodation, programs and services that best meet their needs and preferences. Annually, the Patient Services Coordinator assists in the completion of the placement paperwork for approximately 50 people, arranging case conferences concerning these patients as well.

The person working as the Patient Services Coordinator is shared jointly by Temiskaming Hospital and the Community Care Access Centre. Through this partnership, the Coordinator is also able to ensure appropriate linkages with CCAC services, and refers roughly 300 people a year in this way.
